How to Use This Calculator
- Score each Basic ADL (0 = requires assistance, 1 = independent): bathing, dressing, toileting, transferring, continence, and feeding.
- Score each Instrumental ADL: phone use, shopping, food preparation, housekeeping, laundry, and medication management.
- Review the Katz ADL Score (0–6) and Lawton IADL Score (0–8).
- Check the overall Functional Independence Level to classify care needs.
- Use the scores to guide care planning, discharge disposition, and level of care determination.
