This calculator builds an hourly rate for family or private-pay caregiving from a BLS-benchmarked base rate for the selected care-complexity tier -- $14.50/hr for companionship up to $42/hr for complex medical care resembling RN-level tasks -- then layers on a per-ADL premium ($0.75 per Activity of Daily Living assisted), a per-IADL premium ($0.50 per Instrumental ADL), and a flat medication-management premium. The regional wage multiplier applies only to the base complexity-tier rate, not to the flat task premiums added afterward, so a caregiver's per-ADL and per-IADL dollars stay the same regardless of region while the base tier itself scales fully with regional cost of living. Near this calculator's default inputs, regional wage index still moves the resulting hourly rate more than a single extra ADL or IADL, since those premiums are small fixed dollar amounts while regionIndex is continuous across a 4x range applied to the (larger) base-tier figure -- but over the full declared ranges, the base-tier spread that care-complexity level unlocks ($14.50/hr to $42/hr) edges out regionIndex's own full-range effect by a narrow margin, which is why this calculator does not make a universal "dominates" claim for either input.

Weekly and annual compensation are then derived by multiplying the hourly rate by hours worked and an optional overnight premium at half rate, but the number of caregiving hours per week has no effect on the hourly rate itself, only on total pay; it changes how much someone earns at that rate, not what the rate is. The calculator also estimates what a home care agency would charge for equivalent care, applying a 40% markup over the fair hourly rate, to show the potential savings of family or independent-contractor caregiving. What it does not account for: state-specific household-employer tax obligations beyond the flat 7.65% employer FICA share shown, workers' compensation requirements, or overtime premiums required by some states for live-in or extended-hours care.

Why does the regional wage index change the hourly rate more than the number of ADLs assisted?

Regional wage index multiplies only the base complexity-tier rate, not the flat $0.75-per-ADL, $0.50-per-IADL, or medication-management premiums, which are added afterward at the same dollar amount no matter the region. Near this calculator's default inputs it still moves the rate more than a single extra ADL, since one more ADL only adds a flat $0.75 while moving from a 0.7 to a 1.5 regional index changes the (larger) base-tier dollar amount by more than double. That edge isn't about multiplication order -- multiplying the base rate before or after the premiums are summed gives the same base-tier contribution either way -- it's that regionIndex has a wide continuous range applied to a sizeable figure, while a single ADL is a small fixed increment.

How much does complex medical care pay compared to companionship-level care at the same regional index?

The base rate for complex medical care (ventilator management, IV administration, skilled wound care) is set at $42/hr versus $14.50/hr for companionship and light supervision -- roughly a 190% premium before any ADL, IADL, or medication-management add-ons are applied. That gap approximates the real wage spread between registered-nurse- level home care and unskilled companion care in BLS wage data, reflecting the training, licensure, and liability differences between the two ends of the care-complexity spectrum.

If I negotiate more hours per week, does that raise or lower the hourly rate I should ask for?

Neither -- in this calculator, hours per week is a pure multiplier applied after the hourly rate is already set, so negotiating 40 hours instead of 10 quadruples weekly pay at the same rate rather than changing the rate itself. The hourly rate is built entirely from care complexity, ADL count, IADL count, medication management, and regional index, none of which reference hours worked; how many hours you provide is a separate negotiation from what each hour is worth.

Is it always cheaper to pay a family caregiver directly than to hire through an agency?

In this calculator's model, yes -- it applies a 40% markup over the computed fair hourly rate to estimate the equivalent agency cost, reflecting the overhead agencies carry for scheduling, liability insurance, and administrative staff. But the comparison only captures the wage side: hiring a family member directly also means no agency-provided backup coverage if that caregiver is sick, no agency credentialing infrastructure, and it shifts payroll-tax and workers'-comp obligations onto the family as a household employer.
