- Enter the number of Surfaces or teeth involved for the procedure.
- Select the Anesthesia type (0 = none, 1 = local, 2 = N2O, 3 = moderate oral sedation, 4 = general anesthesia).
- Enter the Regional cost multiplier (default 1.0; use higher values for high-cost markets) and your Insurance coverage percentage.
- Review the Procedure fee, Anesthesia fee, Total estimated fee, Insurance pays amount, and Patient owes to prepare your estimate or pre-authorization.

- Anesthesia type
- 0=None, 1=Local, 2=Nitrous oxide, 3=Moderate oral sedation, 4=General anesthesia.
- Regional cost multiplier
- 0.7=Rural/low-cost, 1.0=National average, 1.3–1.5=Urban/coastal.
- Insurance coverage (%)
- Percentage of fee covered by dental insurance (0% for self-pay).
