This calculator estimates how many distinct vaccines an adult's age and risk profile generally call for, following the broad shape of the CDC/ACIP adult immunization schedule rather than reproducing its full, frequently updated detail -- and that shape has shifted in several ways recently that this calculator now tracks. Influenza and COVID-19 are both counted for every adult, but COVID-19 is no longer a flat "everyone, annually" mandate: ACIP moved general adult COVID-19 vaccination to a shared clinical decision-making (SCDM) model in 2025, so it's counted here as a vaccine worth discussing with a clinician for everyone, with elevated urgency reserved for age 65+ and Immunocompromised adults, who ACIP continues to prioritize most clearly. Td/Tdap, RZV (Shingrix), pneumococcal vaccination, and RSV vaccination are added as age or risk factors cross their thresholds: RZV and pneumococcal from age 50 on (ACIP lowered the routine pneumococcal age from 65 to 50 in October 2024), RZV also for Immunocompromised adults 19+ regardless of that 50-year floor, RSV routinely from age 75 on and risk-based from 60-74 (this calculator uses Immunocompromised as its available proxy for that 60-74 risk band, which the real ACIP risk-factor list is broader than), and hepatitis B for adults under 60.

MMR and varicella (both live-attenuated vaccines) are counted only when NOT contraindicated: pregnancy and immunocompromise are HARD contraindications for live vaccines under ACIP/CDC guidance -- not merely a reason to delay timing -- so Pregnant and Immunocompromised profiles get 2 fewer vaccines in Recommended Vaccines than an otherwise identical profile, reflecting that MMR and varicella are excluded from their schedule entirely rather than just rescheduled. This tool is a starting-point screen, not the authoritative schedule -- always confirm against the current CDC adult immunization schedule and the patient's actual vaccination history before making a clinical recommendation.

Frequently Asked Questions

Does marking a patient as pregnant change the Recommended Vaccines count?

Yes. Pregnancy is a hard contraindication for live vaccines under ACIP/CDC guidance -- not just a reason to delay timing -- so MMR and varicella (both live-attenuated) are excluded from Recommended Vaccines entirely for a pregnant profile, 2 fewer than an otherwise identical non-pregnant profile. Pregnancy Status also raises the separate Urgent/Priority Vaccines count, since ACIP recommends Tdap during each pregnancy.

Is pneumococcal vaccination still only recommended starting at age 65, and does this calculator include RSV?

Pneumococcal: no -- ACIP lowered the routine pneumococcal vaccination age from 65 to 50 in October 2024, and this calculator reflects that current threshold (still recommended at any age for Immunocompromised adults). RSV: yes, this calculator includes it. Per ACIP's June 2024 update, RSV vaccination is routinely recommended for adults 75 and older, and risk-based for adults 60-74 with a qualifying chronic condition -- this calculator uses Immunocompromised as its available proxy for that 60-74 risk band, since the full ACIP risk-factor list (chronic heart or lung disease, diabetes, nursing-home residence, and others) is broader than what this calculator collects.

Is COVID-19 vaccination still recommended annually for every adult?

Not as a flat mandate. ACIP moved general adult COVID-19 vaccination to a shared clinical decision-making (SCDM) model in 2025 -- it's a conversation to have with a clinician rather than a universal annual requirement. This calculator still counts it as a vaccine relevant to every adult profile, but only raises its urgency for age 65+ and Immunocompromised adults, who ACIP continues to recommend it for most clearly.

Is this calculator's vaccine list the same as the official CDC schedule?

No. This tool follows the general shape of the CDC/ACIP adult immunization schedule's major age and risk-factor categories, but the official schedule is updated periodically and includes considerably more nuance (specific product choices, catch-up series details, and additional risk-based indications) than this calculator's count-based summary captures. Always confirm against the current published schedule.

Why does being a healthcare worker add urgency but not more vaccines?

Healthcare workers are typically already covered by the same age- and risk-based vaccine recommendations as any other adult of their profile -- what changes is how soon those recommendations should be completed, given ongoing patient-facing exposure risk. This calculator reflects that by raising Urgent/Priority Vaccines rather than adding new vaccines to Recommended Vaccines.
