How to Use This Calculator
- Answer all five IIEF-5 questions (Q1–Q5) on a scale of 1–5 based on your experiences over the past 4 weeks.
- Enter your age and indicate yes/no for diabetes, hypertension, and smoking status.

- Review the IIEF-5 total score (1–25): 22–25 = no ED, 17–21 = mild, 12–16 = mild-moderate, 8–11 = moderate, 1–7 = severe.

What each input means
- Q1: Confidence in erection (1-5)
- How confident are you that you could get and keep an erection? 1=very low, 5=very high.
- Q2: Erections for penetration (0-5)
- Erections hard enough for penetration? 0=no activity, 1=almost never, 5=almost always.
- Q3: Maintaining after penetration (1-5)
- Ability to maintain erection after penetration? 1=almost never, 5=almost always.
- Q4: Difficulty maintaining (1-5)
- How difficult to maintain erection to completion? 1=extremely difficult, 5=not difficult.
- Q5: Satisfaction (1-5)
- Satisfaction with intercourse? 1=almost never, 5=almost always.
- Age
- Your age in years.
- Diabetic (0=No, 1=Yes)
- 1 if you have diabetes.
- Hypertensive (0=No, 1=Yes)
- 1 if you have high blood pressure.
- Smoker (0=No, 1=Yes)
- 1 if you currently smoke.
- BMI
- Body mass index (weight kg / height m squared).
What each result means
- IIEF-5 total score (1-25)
- Total IIEF-5 / SHIM score. 22-25=no ED, 17-21=mild, 12-16=mild-moderate, 8-11=moderate, 1-7=severe.
- Severity (0-4)
- 0=no ED, 1=mild, 2=mild-moderate, 3=moderate, 4=severe.
- Vascular risk score (0-100)
- Estimated contribution of vascular risk factors to ED.
- Psychological component (%)
- Estimated likelihood that psychological factors contribute to ED.
- Predicted PDE5i response (%)
- Estimated likelihood of responding to PDE5 inhibitor medication.
