How to Use This Calculator
- Enter the child's age in months (6–216) to determine the dentition stage and appropriate varnish dose.
- Set the Caries risk level (1 = low, 2 = moderate, 3 = high) using the AAPD caries risk assessment tool.
- Indicate whether the child has access to Community fluoridated water (0 = No, 1 = Yes) and the number of Active carious lesions currently present.
- Review the recommended Application interval in months, Applications per year, and Dose in mL of 5% NaF varnish per visit.
- Note the Fluoride per application (mg), Annual fluoride delivered (mg), and Estimated annual cost to support treatment planning and insurance authorization.

- Age in months (6 months to 18 years).
- Caries risk level
- 1 = Low, 2 = Moderate, 3 = High (per AAPD caries risk assessment).
- Community fluoridated water?
- 0 = No, 1 = Yes. Absence of fluoridated water increases risk.
- Active carious lesions
- Number of active white-spot or cavitated lesions currently present.
