About this calculator
This calculator splits Total Sites Measured into four buckets -- healthy, mild, moderate, and severe -- built entirely from Sites ≥4mm and Sites ≥6mm relative to Total Sites, since sites at 6mm or deeper are clinically a subset of sites at 4mm or deeper. Sites ≥6mm is treated as capped at Sites ≥4mm for this split, so entering a Sites ≥6mm count higher than Sites ≥4mm doesn't produce a nonsensical result; raising Sites ≥6mm always raises or holds steady Severe Sites %, never lowers it. Within Sites ≥6mm, this calculator further splits Moderate Sites % and Severe Sites % using an undisclosed-until-now 60/40 ratio -- there's no separate real input distinguishing, say, a 6-7mm site from an 8mm+ site, so that split is this calculator's own illustrative heuristic, not a measured value; treat the moderate/severe boundary as approximate. Classification is driven by how widespread deep (≥6mm) pocketing is -- the percentage of all sites at 6mm or deeper -- rather than by Average Pocket Depth, because real periodontal staging (2017/2018 AAP/EFP World Workshop) keys off interdental clinical attachment loss or radiographic bone loss extent, not the average probing depth across all sites.
This calculator doesn't collect attachment loss or radiographic bone loss directly, so it uses the extent of 6mm+ pocketing as the closest available stand-in: no 6mm+ sites and no 4mm+ sites reads "Healthy Periodontium"; some 4mm+ sites but no 6mm+ sites reads "Gingivitis / Stage I"; and once 6mm+ sites are present, how large a share of all sites they make up separates Stage II (under 15%), Stage III (15-32%), and Stage IV (33% or more) -- the same real Stage I/II/III boundary percentages used elsewhere in this calculator's own catalog. Average Pocket Depth itself plays no role in either the four-bucket percentage split or Classification; it's shown only as an informational readout of what was entered. Bleeding on Probing % is simply Bleeding Sites divided by Total Sites Measured, with a standard interpretive band attached (Ainamo & Bay): under 10% reads Health, 10-30% reads Localized Gingivitis, and over 30% reads Generalized Gingivitis. Those "Stage" labels borrow terminology from periodontal-staging concepts as a simplified aid, not the full multi-factor 2018 AAP/EFP classification, which also weighs clinical attachment loss, radiographic bone loss pattern, and tooth loss.

How to Use This Calculator
- Enter Average Pocket Depth (mm), Sites ≥4mm, and Sites ≥6mm.
- Set Bleeding Sites and Total Sites Measured.
- Review the Classification result.
- Use Healthy Sites % (%) and Mild Sites % (%) to inform your decision.
- Use the chart to visualize the results and explore different scenarios by adjusting inputs.
What each input means
- Average Pocket Depth (mm)
- Mean probing depth across all measured sites; informational only, does not drive Classification
- Sites ≥4mm
- Number of sites with probing depth of 4mm or greater
- Sites ≥6mm
- Number of sites with probing depth of 6mm or greater
- Bleeding Sites
- Number of sites with bleeding on probing
- Total Sites Measured
- Total number of probing sites (typically 6 per tooth × 28 teeth = 168)
How this is calculated
Worked example, using the default values
- Identify Input Parameters4 parametersAverage Pocket Depth (mm) = 3.2, Sites ≥4mm = 24, Sites ≥6mm = 6, Bleeding Sites = 30 = 5 input(s) provided
- Calculate ClassificationStage II Periodontitis = Stage II Periodontitis
- Calculate Healthy Sites %Healthy Sites %85.71 = 85.71%
- Calculate Mild Sites %Mild Sites %10.71 = 10.71%

Frequently Asked Questions
What happens if I enter a Sites ≥6mm count higher than Sites ≥4mm?
Since every site at 6mm or deeper is also, by definition, a site at 4mm or deeper, this calculator caps the effective Sites ≥6mm value at whatever Sites ≥4mm is set to before computing the moderate/severe split. That keeps Severe Sites % moving in the right direction even if the two counts are entered inconsistently.
How does this calculator decide which deep sites are "Moderate" versus "Severe"?
It splits Sites ≥6mm 60/40 into moderate and severe for the chart -- there is no separate input capturing exactly how deep each 6mm+ site is, so this calculator applies a fixed illustrative ratio rather than a measured value. Treat the moderate/severe split specifically as approximate; the combined Sites ≥6mm figure is the reliable number.
Is the Classification label the official AAP/EFP periodontitis stage, and does Average Pocket Depth drive it?
Not literally, and no -- Average Pocket Depth is shown only as an informational readout and plays no role in Classification. Classification instead follows how widespread 6mm+ pocketing is across all sites measured (no 6mm+ or 4mm+ sites is healthy, some 4mm+ sites with no 6mm+ sites is Gingivitis/Stage I, and increasing share of 6mm+ sites moves through Stage II, III, and IV), using extent of deep pocketing as a stand-in for the clinical attachment loss or radiographic bone loss that the real 2018 AAP/EFP classification actually stages on. That full classification also factors in tooth loss and case complexity this calculator doesn't collect, so treat the "Stage" label as a rough guide, not a formal diagnosis.
What does Bleeding on Probing % mean, and does it affect Classification?
Bleeding on Probing % is Bleeding Sites divided by Total Sites Measured, with a standard interpretive label attached: under 10% is Health, 10-30% is Localized Gingivitis, and over 30% is Generalized Gingivitis (the Ainamo & Bay convention). It does not factor into Classification, Healthy Sites %, Mild Sites %, Moderate Sites %, or Severe Sites % -- those come entirely from the pocket-depth site counts, kept as a separate readout.
