- The questions cover mood, energy, sleep, appetite, concentration, and thoughts of self-harm.
- Review the Total PHQ-9 Score: 0–4 = minimal, 5–9 = mild, 10–14 = moderate, 15–19 = moderately severe, 20–27 = severe depression.
- Note that a score of 1 or higher on question 9 (self-harm thoughts) always warrants immediate professional evaluation.
