- Height (cm)
- Patient height in centimeters.
- Weight (kg)
- Patient weight in kilograms.
- Protocol dose (mg/m²)
- Drug dose per m² BSA as specified by the chemotherapy protocol (e.g. 75 mg/m² for cisplatin).
- Dose reduction (%)
- Percentage dose reduction for renal impairment, hepatic dysfunction, or prior toxicity.
- Cap BSA at 2.0 m² (1=yes, 0=no)
- Many protocols cap BSA at 2.0 m² to avoid excessive dosing in obese patients.
- Number of cycles
- Total planned chemotherapy cycles.
