How to Use This Calculator
- Review the ECOG performance status scale descriptions: 0 = fully active to 5 = deceased.
- Select the score that best describes the patient's current functional capacity.
- Read the ECOG score and its clinical interpretation for treatment planning.
- Review Karnofsky Performance Score (KPS) equivalent for comparison with older literature.
- Use performance status to guide treatment intensity — ECOG >= 3 often limits aggressive chemotherapy.
What each input means
- ECOG score (0–5)
- 0=fully active, 1=restricted strenuous activity, 2=ambulatory >50%, 3=bed/chair >50%, 4=completely disabled, 5=dead.
- Oral intake (0=normal, 1=reduced, 2=mouthfuls)
- Current oral intake level for PPI calculation.
- Edema present (0=no, 1=yes)
- Presence of peripheral edema.
- Dyspnea at rest (0=no, 1=yes)
- Breathlessness at rest.
- Delirium present (0=no, 1=yes)
- Presence of delirium or altered consciousness.

- 0–5 scale (lower is better functional status).
- Karnofsky (KPS) estimate
- Approximate Karnofsky score (0–100, higher is better).
- Palliative Prognostic Index
- PPI score (Morita et al.): >6 = <3 wk survival, 4–6 = <6 wk, <4 = >6 wk.
- Estimated prognosis (weeks)
- Rough survival estimate based on PPI score.
- Phase I trial eligible (0/1)
- Most phase I trials require ECOG 0–1.
- Standard chemo eligible (0/1)
- Standard chemotherapy typically requires ECOG 0–2.
- Palliative care only (0/1)
- ECOG 3–4 generally indicates palliative-only approach.
