Patient age in years.
Weight (kg)
Patient weight in kilograms for Cockcroft-Gault calculation.
Female (0=no, 1=yes)
Apply 0.85 correction factor for females in CrCl calculation.
ECOG PS (0–5)
ECOG Performance Status. Most trials require 0–1, some allow 0–2.
ANC (×10³/μL)
Absolute neutrophil count. Minimum usually 1.5 ×10³/μL.
Platelets (×10³/μL)
Platelet count. Minimum usually 100 ×10³/μL.
Hemoglobin (g/dL)
Hemoglobin level. Minimum usually 9.0 g/dL.
Serum creatinine (mg/dL)
Serum creatinine for Cockcroft-Gault CrCl calculation.
Total bilirubin (mg/dL)
Total bilirubin. Must be ≤1.5× ULN (ULN = 1.2 mg/dL).
ALT or AST (U/L)
Use the higher of ALT/AST. Limit is ≤3× ULN (or ≤5× with liver mets).
