How to Use This Calculator
  1. Enter the child's age in months (6–216) to determine the dentition stage and appropriate varnish dose.
  2. Set the Caries risk level (1 = low, 2 = moderate, 3 = high) using the AAPD caries risk assessment tool.
  3. Indicate whether the child has access to Community fluoridated water (0 = No, 1 = Yes) and the number of Active carious lesions currently present.
  4. Review the recommended Application interval in months, Applications per year, and Dose in mL of 5% NaF varnish per visit.
  5. Note the Fluoride per application (mg), Annual fluoride delivered (mg), and Estimated annual cost to support treatment planning and insurance authorization.
