- Select the Procedure category (1 = diagnostic, 2 = preventive, 3 = restorative, 4 = endodontic, 5 = extraction, 6 = interceptive ortho, 7 = sedation only).
- Enter the number of Surfaces or teeth involved for the procedure.
- Select the Anesthesia type (0 = none, 1 = local, 2 = N2O, 3 = moderate oral sedation, 4 = general anesthesia).
- Enter the Regional cost multiplier (default 1.0; use higher values for high-cost markets) and your Insurance coverage percentage.
- Review the Procedure fee, Anesthesia fee, Total estimated fee, Insurance pays amount, and Patient owes to prepare your estimate or pre-authorization.
