  1. Enter the child's age in months and Frankl behavior rating (1 = definitely negative to 4 = definitely positive).
  2. Set Procedure urgency (1 = elective, 2 = urgent, 3 = emergency), Procedure complexity (1 = simple to 3 = complex), and Safety risk level (1 = low to 3 = high movement risk).
  3. Indicate whether Special healthcare needs are present and how many Previous failed attempts without stabilization have occurred.
  4. Review the Stabilization need score (0–100) and Recommended type (0 = not indicated, 1 = active/staff holding, 2 = passive/papoose, 3 = mouth prop, 4 = consider sedation/GA).
