  1. Indicate Pulp exposure (0 = none, 1 = mechanical, 2 = carious) and whether Spontaneous or night pain is present (0/1).
  2. Note radiographic findings: Periapical radiolucency (0/1), Furcation involvement (0/1), and Internal resorption (0/1).
  3. Enter the percentage of physiological Root resorption already present and whether Hemostasis is achievable within 5 minutes (0 = No, 1 = Yes).
